The health insurance annex — Anlage SVSource: arbeitsagentur.de — Link last checked: 23.09.2026
Anlage SV – the health insurance annex, filled in
The Anlage SV is the shortest of the Jobcenter annexes — two pages — and it settles something that will not wait: who pays for health insurance while the benefit runs.
Who it is for
The form defines its own scope precisely: it is filled in for every person in the Bedarfsgemeinschaft who is not compulsorily or family-insured in the statutory health and long-term care scheme.
So: anyone covered through work or through Familienversicherung does not need it. Anyone privately insured, or not insured at all, does.
Section C – the question that decides everything
A short set of questions about insurance status, the first being whether the person is privately insured now or was last insured that way. The two possible outcomes are entirely different, and the form spells them out itself:
- Yes to any of them: the person is not compulsorily insured in the statutory scheme during the benefit period, but may claim a subsidy in section D towards private or voluntary statutory cover.
- No to all of them: compulsory statutory insurance applies. And there the clock starts.
The two-week deadline
Where compulsory insurance applies, the form asks you to choose a Krankenkasse and hand in a Mitgliedsbescheinigung or other proof within two weeks, and recommends doing it together with the application.
That is the most practical line in the whole annex. The statutory funds are open to you, the choice is yours, and delay is the only thing that creates a problem.
How the proof is obtained in practice
- Choose a statutory fund. They are obliged to take you, and you may compare them.
- Ask for a Mitgliedsbescheinigung and say it is for the Jobcenter.
- Hand it in with the application if possible, and within two weeks at the latest.
- Keep a copy and something showing when you handed it over.
The health card itself then arrives by post from the fund, not from the Jobcenter.
Section D – the subsidy
For those not compulsorily insured: a claim towards the premium of private or voluntary statutory cover. And the form opens with advice worth stopping at — ask a statutory health insurer whether statutory cover is possible for you, to avoid a financial disadvantage. The form is pointing you at a door that may suit you better than the one you are standing in.
Why the form still says Bürgergeld
The annex is still titled with the word Bürgergeld although the benefit was renamed Grundsicherungsgeld on 01.07.2026.
This is not an out-of-date file. The Bundesagentur für Arbeit says on its own page that the old term still appears here and there, that all forms and online services remain usable, and that decisions already issued stay valid. Fill the form in as it stands; there is no newer one to wait for.
Mistakes that keep happening
- Filling it in for someone already statutorily insured, who does not need it.
- Letting the two weeks pass, so the membership has no proof behind it.
- Waiting for the Jobcenter to pick the fund. The choice belongs to you.
- Staying on expensive private cover without asking whether statutory cover is possible — which the form itself asks you to do.
