Family Health Insurance in Germany – Familienversicherung
One of the better features of the German statutory health system is that a spouse and children can be added to the insured person’s cover with no additional contribution when the conditions are met. This is Familienversicherung, family insurance.
Many families pay separate contributions for no reason, simply because they do not know this route exists or assume it is complicated. In practice it is one form sent to the insurer.
Who can be added?
The idea is that family members with no income, or with income below a certain limit, join the earner’s cover within the statutory system (gesetzliche Krankenversicherung). The limits and conditions are set by the insurer in line with the law and change annually — which is why no figure is quoted here; ask your own insurer.

The official form and the responsible office
The form on the button above is the AOK one, and it stands as an example. Every health insurer issues its own: if you are with TK, Barmer, DAK or another, ask yours for the Fragebogen zur Familienversicherung. The questions and the conditions are the same everywhere; only the sheet differs.
There is no single federal form here: every insurer (AOK, TK, Barmer, DAK and others) publishes its own form on its official site. Take it from your own insurer — the forms differ and the financial limits change every year.
Common mistakes that delay a case
- Using another insurer’s form instead of your own
- Leaving out a spouse’s side income, however small
- Sending the form without the attachments (birth certificate, marriage certificate, proof of income)
- Reporting late when income rises above the permitted limit
- Assuming that cover begins automatically after a marriage or a birth
Frequently asked questions
Is it really free?
When the conditions are met, yes — no additional contribution for the person added. That is the essential difference from individual cover.
My wife works part-time — can she be added?
It depends on how much she earns and the type of work. The limit changes annually, so ask the insurer directly.
Where do I get the form?
From the official website of your own insurer, or by contacting them. There is no single form covering all insurers.
What about private insurance?
Familienversicherung belongs to the statutory system. Private insurance works to different rules.
Please note: The financial limits and conditions are set by the insurer in line with the law and change annually. Your insurer’s answer is what binds.
